Central South University Xiangya Second Hospital Medical Care Oral Psychological Process: Feeling The Stones Across The River To Create Shelter Template Spend A Lot Of Time To Psychological Adjustment Of Patients.
At 17:30 on March 10th, with the closure of the east entrance and exit of Hongshan gymnasium in Wuchang District of Wuhan, the Wuchang shelter hospital ended its mission. After running for 35 days, it was the first cabin in Wuhan and the last cabin of Wuchang shelter hospital.
As one of the first building of the shelter, the medical workers of the national emergency medical rescue team (Xiangya No.2 Hospital of Central South University) experienced the beginning of the "shelter hospital".
After 36 days in Wuhan, Zhang Huilin, deputy director of the nursing department of Xiangya No.2 Hospital, Central South University, slept very late. "I slept more than 8 hours this morning and felt very comfortable." In March 11th, Zhang Huilin told the twenty-first Century business reporter in the Wuchang shelter hospital. After coming to Wuhan, Zhang Huilin usually got up at six o'clock and got the Wuchang cabin hospital at about 7:30.
Zhang Huilin's mood was complicated when he finished the work of the shelter hospital.
"On the one hand, we are very happy that the patients can overcome the disease and leave the hospital. On the other hand, we are a bit reluctant to give up. We have established good feelings with patients. We came first days from the opening of the shelter, when the shelter was still under construction, until now, until the end, in fact, we have deep feelings in the other side of the cabin. Zhang Huilin said.
Zhu Weihong, deputy chief physician of Department of orthopedics, Xiangya No.2 Hospital of Central South University, looks at the experience of more than one month in Wuchang shelter hospital, which takes a long time to recall and digest. "After many years, I may be tempted to recall the many scenes here, some memorable, moving and unforgettable scenes and fragments. People and things are so real, some happy, some sad, some depressed, and some exciting you." Zhu Weihong said.
From strange to familiar
On the way to Wuhan, we knew that our mission was to build a shelter hospital. At that time, no one knew how to build a shelter hospital, especially the shelter for infectious diseases. Zhu Weihong said.
Xiao Kui, deputy director of the Department of respiratory and critical care medicine, Xiangya No.2 Hospital of Central South University, recalled that the time when he received the mission from Changsha was rather blank. However, when I came to the Hongshan gymnasium, I saw the layout of the bed inside.
The first group of patients in the Wuchang shelter hospital was admitted from 11 to 6 in the evening of February 5th at 12 noon. At that time, everyone was basically turning around. It was very intense that no one had ever experienced such a large scale of patients. Xiao Kui said.
Even at home doing countless exercises and psychological preparation, you will be nervous when you really play. The first day after most medical workers came to the shelter, they wore such heavy protective clothing for the first time in their career.
"Wearing protective clothing for the first time took nearly more than 50 minutes to get dressed." Xiangya No.2 Hospital psychiatric nurse Liu Jin said.
Liu Jin was born in 94 years and is one of the youngest members of Xiangya No.2 Hospital in Wuchang shelter hospital. Liu Jin, who first entered the shelter, was a little nervous and scared. "At that time, all the clothes were wrapped very tightly, and the blindfold was also very tight. After entering the room, people began to have anoxia because they talked too much, because they were too hard to breathe.
Due to lack of experience, speaking too quickly leads to anoxia. Liu Jin, who first entered the shelter, stayed for more than two hours. But after that, they gradually grasped the methods, wearing protective clothing faster, speaking more smoothly, and keeping their breathing as gentle as possible.
However, after all, there is a clear difference between the stadium and an infectious disease hospital. Apart from the fact that the hardware has not been completed and the conditions are tight, the medical treatment methods for the patients have also undergone the process of learning.
"The patient was filled overnight and I saw so many patients. I was wondering how we should start our work." Zhang Huilin said.
Xiao Kui told reporters that the first week when he first came to Wuhan was the busiest time.
"When patients come, we have to set up a whole set of files. We have to divide beds, ask for medical history, measure vital signs, record relevant information, prescribe doctors, prepare medicines, which is a series of actions. The first few days' work was very hard. I was only about 5 hours of sleep every day. " Xiao Kui said.
Subsequently, from many medical teams, doctors, nurses and patients began to become familiar with each other. Despite the fact that the shelter hospital is at the peak of the number of patients and the intensity of their work, the daily operation is becoming more stable as the operation between them is more coherent.
With the discharge of patients, the working pressure of medical staff began to decrease, and there was more time for targeted treatment. According to Xiao Kui, as the number of patients slowly decreases, the average time for doctors to communicate with one patient is increased. In the later stage, doctors can have 5 minutes or even 10 minutes on average when they go to the ward rounds, and communicate with each patient fully.
"There is no template to build a shelter hospital, and the Wuchang shelter hospital is the first batch of Wuhan built and used. We are" stone "crossing the river by feeling the stones. After that, we learned from and learned a lot from our shelter. Xiao Kui said.
From doctor-patient to friend
"The patients in the shelter hospital have basically no symptoms, but they are confirmed to be new crown pneumonia. Admitted to hospital, one is to let us carry out close observation, treatment, care, and the two is to play the role of isolation. Zhang Huilin said.
Zhu Weihong said that for mild patients, the most fundamental core issue is to overcome disease through treatment. The main feature of Xiangya No.2 Hospital's medical team is to pay attention to the treatment of psychological intervention under the epidemic situation, and to care for patients and strive for understanding, so that they feel respected.
In addition to being a nurse in the Department of psychiatry, Liu Jin is also a psychological consultant. The first patient he contacted was an insomniac.
"Because of this sudden illness, he has anxiety and worry. I can't sleep every night. I can go to bed at four or five in the morning and wake up in one or two hours in the morning.
Later, Liu Jin told the patients some ways to sleep, through sleep therapy, music relaxation, mindfulness therapy to guide patients. After three or four days, the patient's condition of insomnia gradually improved.
In fact, there are not a few patients who are worried or worried about their illness or family in the shelter hospital.
In view of this situation, they set up a psychological group to carry out small group therapy with warmth and relaxation as the theme. In addition, the medical team also set up a psychological wall of hope, so that patients can express their wishes, and through some painting treatment, build confidence for them.
Zhang Huilin told reporters that in the shelter hospital, more time is needed to help patients relieve psychological stress.
"There was a hairstylist who gave his sick friend a haircut in the shelter. We thought he was happy, but he was very worried that no one would get his hair cut after discharge. Zhang Huilin said.
Xiao Kui believes that the difficulty of treatment for mild patients is that some of them are unable to get rid of nucleic acids, which will bring psychological distress to them.
"These patients need more medicine than psychotherapy. Let him realize that "sickness comes like a mountain, but sickness goes away." Xiao Kui said.
Zhang Huilin said that many patients and doctors and nurses have changed from common "doctor patient relationship" to "friend" relationship, adding WeChat to each other. After the end of the epidemic, many patients expressed hope that medical staff would have the opportunity to visit Wuhan again, hoping to have the opportunity to see the appearance of the respirators.
Although the historic mission of the shelter has been completed, the front-line work of "shelter medical care" will continue. "After the cabin, we will be standing by, waiting for the next task." Zhu Weihong said. (Editor: Bao Fang Ming)
